For radiology programs that want three letters, I was advised to assign two from radiologists and one from an attending from a third year rotation. Was this good advice? Since one of the letters isn't in, I could reassign if needed.

How do applicants usually do it when they want 3 letters? 2 rads and 1 3rd year or 1 rads and 2 3rd years?

Our interest group just had a meeting with the program director, and he basically said that it's more important that you have a descriptive and very strong letter from any physician than a form letter from a radiologist. If you have two from radiologists that honestly had the time to get to know you then that would be great, but it's simply not true that you need to have even one.
 
What you should aim for is well written letters, from attendings who know how to write and describe well (i.e. look at how they speak on rounds and write their notes), who know you reasonably well, and who come from a variety of fields.
