Letters of Reccomendation

I know that for pharmacy school applications, I have to have recommendations from professors, employers, etc. I am confused about whether they are supposed to write a letter of recommendation, or if they fill out a questionnaire about my qualifications. I have heard a mix of answers and wanted to clear this up?
