I have done a lot of locum jobs and currently applying for a state license. They are asking me for my employers address and my work location. Would the employer be considered the Locums company, and work location will be the hospital that my shifts were scheduled at?

If you have done a lot of locums, occasionally you end up picking one or two shifts at a sister facility. What are the repercussions of making an honest mistake and forgetting to mention a facility that you may have only done one or two shift at, but had privileges cause it was a sister facility of the hospital where you did bulk of your shifts.

the employer would be the main hospital...if you had to credential separately for the sister hospital, then you will need to list them separately, otherwise the one address should be fine. The licensing board is just looking to get verification of your credentials.

I've been working locum tenens for awhile and used to struggle with this part as well. What I ended up doing was just writing "self-employed" and use the hospital address where I currently work the most.

I would not recommend to do that, even though it's such a pain to fill out all those forms.

I have worked at a lot of different facilities, some as a locum psychiatrist.....and usually during my career worked at more than 1 position at a time. I listed everyone when I applied for my Arkansas license in 2016
