What type of optometrist license do you need to travel and help homeless people or people in old age homes?

The reason I ask is because currently all states make you have a license for each location of practice.
 
You will have to get a license in each state that you want to do this. Some can get around this if you are under the auspices of a medical school and practicing at a medical school. Even optometry schools require their faculty to have a license in the state of the campus.
