Do they mean alternate names only as they pertain to your diplomas and professional credentials?

I was adopted at age 17 by my stepdad and changed my last name. Do I have to list my birth name as an alternate name and provide the court documents? I don't see why they would care what my name was when I was 16.

I think it pertains to running a background check, though since juvenile records are sealed I'm not sure what they would find anyway. You would be suprised how many criminals try to change their names and continue conning. I would list it just to be completely honest knowing that they will not find anything.
