The state where I will be working towards licensure requires that individuals accruing postdoctoral hours apply for a temporary license. The application for the temporary license asks for five reference letter forms, one of which must be from my doctoral committee, two from licensed psychologists, and two from other folks I've worked with professionally (who can be, but don't have to be, licensed psychologists). I'm planning to get letters from my dissertation chair (who was my mentor throughout graduate school) and two from my internship supervisors. I'm not sure who I should ask for the two other letters.
Throughout graduate school I did five practicum placements at community sites, so I have a myriad of supervisors who could write letters. However, I was only at each site for 9-12 months, so I'm wondering if it makes more sense to have letters from licensed psychologists in my department (e.g., my DCT) who have known me for longer (~6 years). The downside of that is that none of them have directly supervised me.
Of note, the letter of recommendation is more of a form than anything else, but does ask for amount of time and in what capacity the recommender knows me. Most of questions are simple and closed-ended (e.g., ethical violations, moral character), but three questions allow for open-ended responses (evaluation of knowledge/experience in the practice of psychology, recommendation for licensure and why, and any additional comments).
