I currently work as a resident in Michigan. I plan to get a job in IL this upcoming year and want to pursue licensure now.

I have a question regarding the site/process for anyone who can help

http://www.idfpr.com/dpr/apply/physli.asp

There are several options and I am not sure which one applies to me. I passed Step 3 about 2 years ago and I hold a temporary/ education licence (whichever one redidents have) in Michigan.

The options are:

Physician - Licensure by Examination
These instructions cover the basic requirements and procedures to follow for applying for a license as a physician to
practice medicine in Illinois. These instructions cover licensure requirements for examination applicants only. If you
are applying for licensure by acceptance of examination, endorsement or restoration, you must request a different
application.

Physician - Licensure by Endorsement

These instructions cover the basic requirements and procedures to follow for applying for a license as a physician to
practice medicine in Illinois. These instructions cover licensure requirements for endorsement and acceptance of
examination applicants only. If you are applying on the basis of endorsement you
MUST BE currently licensed to
practice medicine in all of its branches in another jurisdiction. DO NOT use this application packet if you wish to apply

under the licensure method of examination in Illinois or restoration.

Physician - Licensure by Acceptance of Examination
These instructions cover the basic requirements and procedures to follow for applying for a license as a physician to
practice medicine in Illinois. These instructions cover licensure requirements for endorsement and acceptance of
examination applicants only. If you are applying on the basis of endorsement you
MUST BE currently licensed to
practice medicine in all of its branches in another jurisdiction. DO NOT use this application packet if you wish to apply

I am seeking a permanent license. I will certainly call but I know that the State of IL Dept of Regulation is notoriously hard to get a hold of.

Thanks in advace

I think you want to do "Licensure by Examination". I believe the other two options are for ECFMG holders. You would then submit documentation of your Step3 pass along with the packet. You should definitely still call in just to be safe and prevent your licensure from stalling. Hope this helped somewhat! 🙂

Licensure by Endorsement applies when you have a full and unrestricted license in another state and are applying for a license in a new jurisdiction. Some states have reciprocity. This is not soley for ECFMG holders, as noted above.

Since you do not have a full and unrestricted license, I believe you want to apply for Licensure by Examination (as you have completed Step 3) but as noted above, calling the state would be a good idea, regardless of how difficult it is.

thanks for the replies. i'll call as soon as i get a chance.

I was searching the forum for info for myself but then I found this info in the paper application:

Following are definitions of the various methods used in issuing licenses for professionals in the
State of Illinois. Some of these licensure methods may not be applicable to your profession. Refer
to the enclosed instruction sheet to determine the specific licensure methods/requirements for your
profession.

Licensure Methods Definition

Examination Applicant has applied or is required to take and pass all
or a portion of an exam scheduled and/or given by the
Department or a representative of the Department.

Endorsement of License Original license issued in another state and that state's
requirements were substantially equivalent to Illinois
requirements at time license was issued.

Acceptance of Examination Applicant has taken a National Exam, referred to by
Illinois statute, in any state. Applicant may or may not be
licensed in another state.

Restoration Applicant has previously been licensed in State of Illinois
and has allowed license to lapse long enough to require
reapplication. Possible exam passage and/or committee
review.

Grandfather/Waiver Applicant will be licensed without regard to current
requirements because statute allows this based on past
qualification and practices (for a specified time only).

Non-examination Applicant is licensed by meeting qualifications required
by statute. There is no exam for these professions.
These can be either businesses or individuals.

Does anyone know how IL works their expiration dates? In Ohio your license expires on a set date based on the first letter of your last name and not based on when you received your license.
