Linezolid neuropathy

Hey people , does anyone know the mechanism of neuropathy / Serotonin Syndrome with Linezolid .. It just seems like a completely random side effect for an antibiotic ( except if it is malabsorption related but i dont think so.. ) and I hate memorizing so I would appreciate if anyone explained it!

Linezolid is a weak MAO inhibitor (I believe it was initially developed for this purpose). MAO breaks down serotonin.

No one really knows why any drug causes peripheral neuropathies. Wikipedia currently says a theory is that mitochondrial toxicity is the cause, but I don't think I'd put my money on that yet. Sorry you'll just have to memorize that one.
 
Linezolid is a weak MAO inhibitor so it increases serotonin concentration so taking this antibiotic with other pro-Serotonin will lead to a serotonin syndrome. It's mechanism of neuropathy (optic neuritis) has not been clear but I'm sure some online med journals might give alittle explanation

Myelosuppression ( anemia,leukopenia, and thrombocytopenia) is more common than peripheral neuropathy and optic neuritis which is only seen in pts on long term Linezolid therapy (>6 wks).

I always used the acronym HOT: High risk of serotonin syndrome, Optic neuritis, Thrombocytopenia

(That's right, I'll take credit for how amazing that acronym is)

Nice, didn't know the MOA of linezolid. Just had it written in my notes in the same section as serotonin syndrome..one more thing I can actually understand..
