List of Programs With Supplemental Applications

Does anyone have a list of all the programs that have supplemental documents. Here is what I have gathered so far: If there are others please post
U Arizona
Cornell
San Diego
UC Irvine
George Washington U
Georgetown
U Florida Jacksonville
U Texas San Antonio

Tulane
 
Hey great list. This was very helpful. I applied to some of these and didn't note these supplemental materials. Thanks.

If anybody knows of any more please post.
 
Does anybody know what the hell is wrong with Tulane?? Their secondary is brutal. It's worse than the CAS.
 
Supplemental applications are a way for the program to screen out candidates. The thinking is that if you really are interested in the program then you will complete the supplemental application. You better be really interested in Tulane to fill out that crazy supplemental application.
 
So you fill these out without them asking you to, right? I haven't sent in a few, but Arizona sent me an email asking me to fill it out.

Do all programs send such emails, or do most just expect you to go to their website and figure out if you need to see a secondary?

I don't know if they all mail them to you. Arizona did, I'm not sure about the rest. I really don't know if we should just send the stuff in or wait for an email like the one Arizona sent out. Anybody have an idea?

To find this info, go to the programs home website and try to find the link for how to apply to their residency, or something like that. It will usually have a blurb about what you need to do.

What I've found so far, is that a few programs want your CV, San Diego wants a paragraph or so about why you want to go there (Mine was just three words: Weather and Beach!!!), Kentucky wants a recent eye exam (wtf???), and George Washington wants a page or less about why you want to go there (that should be fun). Some want pictures as well.

Which email address did you use? I sent mine in to the one indicated on SF match and the email did not bounce (however I also did not receive any confirmation that they got it).
 
I sent it to the email indicated on the UC Irvine website under FAQ and it is a different email from the one on SF match website.

UC Davis too.

SF Match site has secondary materials listed for some so make sure to check all the schools you applied to.

UC Davis? Where are you seeing that? On their website, all it says under application process is:

Your application is considered complete when it contains the following:
Only Central Application Service (CAS) applications will be accepted
College transcript
Medical school transcript
Detailed National Board scores with sub-scores, and/or equivalents such as FLEX, VQE, ECFMG
Three letters of reference
Dean's letter. (These are usually mailed after November 1
The University of California, Davis will not not accept H1-B visas for post-graduate trainees
Selected candidates will be contacted for a personal interview. Interviews are scheduled only on selected Friday mornings in November or December.
Applicants will be requested to come to attend Ophthalmology Grand Rounds which are held Friday mornings, prior to interview sessions.
A flexible or mixed medical internship is required prior to entering the residency program.
State licensing requirements mandate training which includes four weeks of family practice in the medical school curriculum.
Acceptance to the residency training programs contingent upon completion of an ACGME accredited PGY-1 year prior to entering ophthalmology training at the PGY-2 level.

Cornell also notifies you by email. They want a "why cornell" paragraph by Sep 7
 
There was a similar thread last year on this topic too. By the end, it was comprehensive and included all the programs with supple stuff. Please search for it, it'll be quite useful I suppose! Its the same programs every year I believe, but dont quote me on that...
 
UAB requires a photo and recent eye exam (Only after an interview has been offered, however)
 
If we can get this added to the list - got an email from Loma Linda today confirming receipt of my CAS app and asking for a response to the question: "Why have you applied to Loma Linda University Department of Ophthalmology?" It's unclear how long they want it to be, but probably a paragraph up to a page at most. Has to be in by 10/15 at the latest.
 
For our eye exams, do you think they can be un-dilated??
 
I think un-dilated is fine -- San Antonio has a sheet they make you fill out, and all they want is acuity/color vision/stereo vision. I've also heard of some places busting out a stereo vision plate on the actual interview day to make sure you have good stereopsis, although not sure if they still do that anymore. No slit lamp exam necessary...
 
I didnt think that we had to resend the letters again personally. I think they just want the names of the letter writers. Did you hear otherwise?

I could be wrong, but that's how I interpreted the Tulane application.

"References: List the names of three person with whom youhave had professional affiliation or training at some time during the past five(5) years and whom you have requested to send letters to this Department. Include, where applicable, director ofresidency training program or of graduate education or fellowship: chairman ofdepartments in which graduate or professional work was taken, or employmentsupervisors."

But maybe that means sent to the department via CAS?

Maybe it's worth a phone call.
 
Yah I assumed they meant letters through the CAS.. didnt really think about it your way.. It might be worth a phone call

People may want to add Washington University to the list as their website states:

"Although we do not require supplementals to the CAS application, you are welcome to send a CV, additional recommendation letters or published articles to the attention of Mary Hitt, Washington University in St. Louis School of Medicine, Department of Ophthalmology & Visual Sciences, 660 South Euclid, Campus Box 8096, St. Louis, Missouri, 63110."

I have not received an email from Tulane stating the supplemental can be sent via email rather than mail. Do you think I should go ahead and email it? Also, what are people doing about Tulane's request to have the last page of their application signed in ink if they email it?

LeLu can clarified where you found that UC Davis requires additional information? I am only finding the description that golfman copied.....

What did UTSW send for supplementals? Did u already receive an interview invite or are u referring to their email saying they will request supplemental information when interviews come out?
