List of work places in the application process?

Hi all.
As many of us the non traditionalists have worked for various employers, when/where do we mention the employment history? On the application? During the interview do we bring a resume? How does that work?
Does background checks reveal employment history?
The reason I ask is that I've worked for quite a few number of companies....(all healthcare related...about 10 which includes side jobs, short term jobs and full time jobs) and I haven't kept track of all the companies I've worked for. I don't want to omit employers when background checks would show all my previous employers!
 
There is room on the primary application for this. I listed all of my past/current employment on there. I believe there was even a specific catagory for health-related work. I did fill out both TMDSAS and AMCAS, so those appliations are blurring together in my head.
