Listing LOR from joint dental practice?

Hi, everyone ... I have a quick, relatively simple question. Two dentists are preparing one letter of evaluation for me, as they currently operate a joint practice. However, the application has space for only one name in the LOR form. Do any of you know if it's possible to indicate that a LOR is a common letter from two individuals (as opposed to being a committee letter)? If not, otherwise I assume I should just list one name ... Thanks!

I had the same experience.

I just listed one dentist in the letter and the one that i listed did something similar to what a committee letter would do:

"I think X, Y, and Z. Doctor Smith agrees and said this . . . ."
