Listing Shadows all in one Experience

How do people list multiple shadows in one experience? Do they leave the name of the organization blank?Supervisor blank?

You can't leave the experience dates blank and my shadowing was at random dates. Should I just list each shadowing experience separately? Though I was thinking that was too much since I have five different shadowing experiences.

I listed mine separately. I had around 30ish hours. Wrote a brief few sentences summarizing my take away from each experience. I kept it short and sweet. So I could talk about it during an interview. Fingers crossed.
