LOE Question

This might be a really dumb question, but is there like a particular form that the evaluator fills out for the LOE? I have the matching forms, but in what format is the evaluator supposed to fill out evaluation? Is it just like a page-long response about the applicant or is there some sort form (kind of like undergrad letters of recommendation) that they use to supplement the evaluation?
