So I'm wondering what the career development possibilities are after getting settled into a job. Because just seeing endless assembly lines of patients doesn't seem sustainable to me. (especially with all the political forces at work seeing to decrease payment). Throughout school and residency, there's always something to shoot for, the next goal, the next achievement.

But after that????

I'm thinking of the doctor equivalent of, say, an accountant who starts out as a Jr Associate, and who, by improving his or her skills, can gradually move up the ranks to Sr Associate, into a managerial role, and then into roles that include helping the firm grow (such as director, managing director or partner). Once partnership is achieved, there are opportunities to help land accounts, go after bigger accounts, improve the firm's profitability, etc.

But I don't see the equivalent path in medicine post residency/fellowship. Since the revenue generating activity is the patient assembly line.. what else is there to aspire to? What new challenges are there available, especially ones that can increase the physician's compensation?
 
Own practice, Research and Development, Head of Department, CMO, CSO, CIO, COO, CEO, politics, health policy, public health initiatives, government work, voluteering
