Looking for a Pre-med manual

Last year reading SDN I found this 70 some page pre-med manual in PDF format. I thing it was from University of Nebraska or maybe New Hampshire. I saved it on my hard drive, but my laptop died. I can't seem to find it anymore. It was very useful as it showed several examples of succesful LORs. Can anyone email it to me or send me a link.
