Hello. I have a newbie question.

I am trying put in a letter of recommendation request in ERAS. Under Author Name, do I put in the person's name and degree or just their name? For example John Smith, M.D., Ph.D. or just John Smith?

John Smith, MD or Dr. John Smith
 
John Smith, M.D. or John Smith, MD
John Smith, M.D. Ph.D. or John Smith, MD PhD
John Smith, Ph.D. or John Smith, PhD

Avoid "Dr John Smith" as it is unclear what the degree designates.

Dentists are referred to as doctor/Dr.

Some nurses with doctoral degrees in nursing call themselves doctor as well.
 
Sorry to bump this old thread but I have a question. I forgot to put md or dr for one of my letter writers name. It was a hassle to get through the medical education department of the hospital so I don't know if they will reupload it. Is it worth sending if I can't get it re uploaded and the authors name just appears as "john doe"
 
I don't think it's a big deal as the letter readers will see the letter writers credentials whatever they are on the actual letter itself.
