LOR for Neuro (categorical and advanced)

I am an US-IMG applying to neurology programs. If there is a program that I can only send TWO (2) letters of recommendation, I don't know what combination of letters to assign:

1) LOR from US rotation (neuro)
2) LOR from peds (great letter geared to neuro)
3) LOR from IM (great letter)

Should I use the US LOR and LOR from peds, or LOR from IM for advanced and categorical neurology programs, respectively? Thanks for your assistance...
