Hi,
I was wondering if there was a form for the letter of recommendations? I know most schools don't have a form but the professor that is writing my letter seems dead-set on having some type of form.

I second that motion... I'm about to start asking professors, and I don't know where to start... is there some kind of form?

There is a form that must be attached with the letter of reference, you can download and print it from this link:

A form also must be attached to your transcript(s) and can be found here:

Although their are no specific guideline on how the professor(s) should write the LOR, but I might suggest that you supply them with a summary of your personal activities, resume, transcript, and maybe a rough draft of your applicant essay (why you want to pursue a career in dentistry), so that they are able to expand into areas other than your academic performance, namely your characteristics, motivation, etc... (this should also make it easy for them to write your LOR)

can the instructor send your LOR before you send in your AADSAS application?

Yes, you can send the LOR before spending your application (probably this is the best method to do because you don't know how long it will take the professor to write the letter). That is why it is so important to attached those supplimentary forms which I have indicated so AADSAS will know who the LOR is for. When they receive the LOR, AADSAS will make a file for you so that when you finish and submit your application, they will have the LOR and transcript ready and can then process your application immediately.

So there is no set form that would help my professor write a letter? He made it seem like such a form was available but I don't know where since I know most schools don't have one. I did give him a list of my activities, why I want to be a dentist etc. so hopefully that will be enough.

^^^ Another thing that helps letter writers are if you give them a copy of your application essay if you are comfortable with that.

No, there is no set form that is issued by the schools or AADSAS. So the professor can write whatever they want on the LOR. It is good tht you provide as much information of you as possible to the professor so that he/she will be able to write stuff other than your academic performances.
