LoR forms with ranking grid vs. without ranking grid

My letter service offers both types of forms. Do medical schools ever request a ranking Lor? Just FYI ill be focusing more on the mid to low tiers.
 
If your institution offers multiple types of official letters, I would just pick the one that makes you look the best.
 
My institution does offer all types but I've heard the type of letter can vary based upon the school. I was just wondering whether it would help or distract if I were to get one with a ranking grid.
 
I'd guess that depends on what you're ranking is. If you're the best student ever to walk through the office, having a great rank will definitely help emphasize that. If you're pretty standard, then maybe the non-ranked letter would be better. However, I have literally no experience with how adcom's evaluate letters, so definitely take this with a grain of salt haha.
