Did anyone here need a Buckley waiver form to get a letter from a dentist? I can't remember if Aadsas gives us these forms to give to our dentist.

What the hell is a Buckley waiver?
 
Sorry, but I have no idea what a Buckley waiver form is! But that didn't stop me from getting a letter from a dentist!

The form that says whether or not you, the applicant, are allowed to read the letter or not.

Well then the answer to your question is no! You do not need one in order to get a letter from your Dentist! And no, AADSAS does not provide such! Hope that's what you wanted to hear.

Yep, you are correct. Just check the box that says you don't want to view the letters.
 
There's a cover letter in fill out? I don't think Aadsas provides that.
