I was reading about the application process on http://www.adea.org/dental_educatio...nts/GeneralInstructions/Pages/Evaluators.aspx


and noticed when we are adding the evaluators we have to add

their email address to send the link so they can attach the LOR
but we also have to write down their home address?

Not sure what you are asking, but you need to have contact information for each letter writer. For a professor, a work/office address is what you can include.
