LOR matching form: address info

hm okay.
i have one professor whom i dont know the school address for (what do they need it for anyway?) and so i have been waiting for her to respond to my email about the address. But for my other two matching forms, even if i did write an address, it didn't even appear on the matching form. Should i just go ahead and put down an approximate address since it's going to be blank on the matching form?

I'm surprised that your school's website doesn't have a directory listing for the professors. I would put down some generic address for the school or maybe the department. I don't think it really matters, you can always change it later.
