LOR Outline

I met with an English professor and I asked her for a LOR which she agreed to write. She asked me to bring some sort of outline, or a list of what adcoms look for in a letter of recommendation, which qualities they want to know about etc.

So I was wondering if anyone can recommend or let me know what a solid LOR for a medical school committee should look like and what should be included.
