LOR Professor from last semester

I took bio 1 and got a C and retook it last semester with a different professor to earn a B. I asked him at the end of the semester if he was willing to write me a letter for when I potentially apply to DO programs in 2015 and he agreed (but he admitted that the best letters come from classes where you earn an A, but he added that I was a good student and I worked very hard in his class)

I haven't seen him since that meeting in January. I'm moving back to my hometown after finals this month, so I may never see him in person again. I may be overthinking this (I get real nervous about these sort of things) but what do I do from here? I know that I need to provide him with a CV or resume and make an interfolio account or something. I just hate feeling like I'm only showing up to get a letter but I also don't want to look like a brown noser.

I also want to mention that I'm in the middle of trying to decide whether medicine is right for me or not....so there might even be a chance I might apply to a different program ie PA or nursing, in which I'll need a letter. Should I mention to him that I'm not solid on my decision to go to med school?

Sorry I think I have some form of social anxiety so I get real worried about these things. Please help a girl out!
 
I was in a similar situation, even thought it was a different subject. I asked in person last semester and the prof said yes. After ~3 months, I sent an email asking if she was still able to write me a letter. After ~1-2 weeks, I got an email saying she would be happy to. She said to email her or come see her at her office hour. I chose the office hour option of course. I gave her my resume and instructions to submit the letter to my school's health committee in a folder.
She said she will get it ASAP in ~1 week.
So basically I:
1. Asked in person. Got a "yes"
2. Emailed to confirm since it has been a while. Got a "yes"
3. Gave resume/info in person to seal the deal.

I don't know if it will work with your prof but it worked with mine.
Reason I emailed in 2. was to avoid awkward situation... I'm just not that into awkward moments. I was thinking "what if she forgot? what if she forgot about me? what if she says no?"

Those are the exact same thoughts that are running through my mind! Bio 1 is such a huge class and I'm just one in hundreds of students. Thanks so much, this is a great approach and I'll be doing this. 🙂
