So I put my professor's name and email address on the application, which sends them an email with instructions to upload it. Does the email contain some kind of form that he needs to follow or do I need to give him a separate form and hand it to him?

If you're doing electronically, there's no further form for you to hand them. If you're doing paper, I believe there's a matching form for you to print out and also give him.
