anesthesia letters are obviously relevant, but beyond that the most important thing is that they be from people that worked closely with you. An IM chair letter is only relevant in as much as it'd be from someone that worked with you and can speak to your character and work ethic. If they can, it's perfect. But the department chair part doesn't carry much cachet in and of itself.
