LOR Question

I am applying to five schools and I would like to know if I need to print out five AADSAS forms to send to each Evaluator? Thanks.

Each evaluator has to fill out just one LOR form when they write your letter. This form will be included in the envelope along with the letter which they send to AADSAS.
