Yes.

-AT.

No. Many program directors can get annoyed to see their own letter. They already know what they wrote about you. By using their letter, you are showing that PD that you could not find another person to support your residency application. Just my 2 cents.

It's totally fine. Let's be honest here -- if the PD wrote you a letter, then they already know all about you. What someone else wrote isn't going to mean very much.

In my opinion, you'd be crazy not to include the letter.

First off, the program director may not remember writing you the letter or what he wrote in the letter.

Second of all, there are many other people who review your application and who have no idea that the PD wrote you the letter. So for them to see that he wrote you a letter with help you alot...

Depends on the residency. Do you know if its a group of people that select/interview you, or is it just that PD? I'm doing anesthesia and I know at my institution my PD rarely talks with the other attendings, and those attendings are more or less in charge of the selection/interview. It would be nice for them to know what the PD thought just in case he didn't go out of his way to describe you. You have to remember, your selection is going to be a group process, even the educational coordinator and secretary have some sway.
