I just want to verify if the LOR is a written letter by the instructor or is there a form that the instructor fills out?

You should ask your adviser this question. I'm not sure how it is for other schools (probably the same), but for my pre-dental department, I have to sign a form to waive my right to read the letter and the person writing the letter has to sign it as well. Contact the pre-dental department or your adviser to make sure of what you need to do.
