LOR recommender forms?

Do most MD/PhD programs require a separate LOR recommendation form like Northwestern below?

That would be a b for every school (something more to worry about making sure it gets in and another added pain for the writers...), but from what I gather by scoping out websites most don't require separate form for LOR writers?

How would something like this be dealt with if you were using interfolio?

yeah, Mayo does, but I know Northwestern doesn't care. For schools that do have a form, ask them if it's required if your school's premed committee has an alternate form that will be included (that's how mine worked). Usually they're ok with it, some want their own form. If they want their own form, send it separately (just mail it) once the school has requested secondaries.
 
Wow, you got me from 2002. Strong work.

You know it's funny. Pitt in particular assured me that those forms were absolutely required for my application to be considered. So I bit the bullet and ended up going through a lot of hassle to track down my recommenders for those forms, one guy not even being in the country and having to fax them back to me!

Another guy my year didn't submit those forms. He was accepted to several programs requiring those forms, including Pitt, that I was not accepted to. $#%@#$%@$

So in other words, who knows how optional these things are. Keep in mind as well that 2002 was a long time ago.
