Do any of the Anesthesia residencies require more than three LORs? Are there any that specify who they should be from? (Beyond having someone in the field write one of them) Thanks for any help!

Most if not all of the programs I have applied to require 3 letters with at least one being from an anesthesiologist. ERAS gives you the option to choose up to 4 LORs to be sent to your selected programs. If you're interested in a program visit their website and look under the application requirements link. Generally, they all require the same things. Good luck!

Thanks for the response, I appreciate it. The only one that I've found that's different is Albany:

3 Current Letters of Recommendation (At least 2 from Anesthesiology Electives)

The only one I've come across is UNC. 3 LORs with the application including a Chair's letter. I'm not exactly sure if the Chair's letter is for anesthesia or for medicine (like the letter for prelim/transitional years possibly?). Anybody have some insight?

Look man, when you start writing "LOR" in anesthesia forums and you aren't discussing epidurals, it is very confusing.
