LOR Template?

Has anyone used a letter of recommendation template that they gave to the docs writing them letters?

One attending asked me if I could send him a template form. I cant seem to find one on my schools website.

The ERAS cover form has some instructions for the letter writer. If you haven't already, you should give it to them. As for "templates" no I never used one but perhaps you could find a general recommendation letter guide on google or something.
