It has been over a month since I asked my professor for a letter of recommendation, which he agreed to do. In addition, it is past the deadline for the committee organization of my school and he has not yet sent it in.
What is the best/kindest way of asking my professor to speed up the process? I dont want to come off as pushy and a burden.
How can I email him and ask him politely? Specifically?

Just be honest. Say that you were wondering how your LOR was coming along, that you found out that it's past the deadline and his letter wasn't there. You can say that you were worried that it got lost on the way to the other office or something so that it doesn't seem like you are placing all the blame on him.
 
She already knew it was past the deadline when I told her to write me the letter
How should I ask her exactly?

How long does should I give a professor to complete the letter?
After receiving all the required materials, how long does a professor need?
 
I'd say at the very least a week, preferably a few weeks. They'll probably finish much quicker than that, but it is rude to leave something to the last minute and expect somebody else to rush to make up for it, especially given how busy most professors are.
