I don't have a pre-med committee to write a letter so I'm using individual professors to meet the LOR requirement (my advisors just hold/send letters for us in Arizona--suck). I'm wondering whether anyone else in this position knows if the generic LOR (their selling me in a 'vote for this student' letter format on professor's letterhead, etc.) is acceptable OR if I need to send each of the professors OSU's 'pre-professional evaluation' form from the secondary app. This might be a stupid question and I should probably just do it...it's just going to be hard to track down all the profs again. Any advice?

Step 3: Letters of Recommendation

Letters should be sent directly to OSU Center for Health Sciences from the evaluator. The student may choose between the two tracks below:

Track 1

* One letter from a college or university pre-professional/pre-med committee
* One letter from an osteopathic physician (D.O.)

Track 2

* Three letters preferably from faculty (2 science and 1 non-science faculty)
* One letter from an osteopathic physician (D.O.)

1. D.O. Evaluation Letter of Recommendation #
2. Pre-professional Evaluation Letter of Recommendation #
3. Applicants may submit Letter of Recommendation or Evaluation forms or both.
