Are you in Atlanta? Do you go to Morehouse or Emory? Why don't you go over the the rehab program over at Emory and ask them? I think the assistant to the program is Miki DeJean. They may be able to give you more insight. And yes you apply for PGY1 (prelim medicine/TY/etc) and PGY2 (pm&r) at the same time in your M4 year.

While more competitive in the last 1-2 years, I don't think pm&r is at the levels of ENT or rads but not as easy as FP or IM; you might be good to go if you just apply more broadly and don't focus only on desirable geographic locales. An away rotation may also help you as well as good grades/narratives (whatever system your school employs) during your M3 year and impresive LORs from PM&R attendings when you submit eras.

You definitely have a chance at obtaining a residency. However, if your scores are low it would be helpful if you emphasize your strengths in other areas. I have a low step I score and a low class rank but I have received many interviews so far (I am currently a fourth year). Many of the programs that offered me an interview are highly ranked and/or well know such as Kessler, Rusk, and Hopkins. Do not get discouraged solely because of your numbers.
