I am currently a second year medical student who is planning to take the USMLE Step 1 this upcoming July and just activated UWorld yesterday. I have since completed 150 randomized questions and scored a dismal 41%. Granted I have not done any dedicated studying for step 1 prior to this (other than my usual coursework) and have only been taught part of pharmacology, but I was honestly expecting to do significantly better. While I admit I may not be a particularly strong medical student, this practice session puts me over 20% under the average of all students who have completed my questions. Is this a major reason for concern at this point or should I expect my performance to improve as I begin to review for this exam?
 
The average you see on UWorld takes into account people who've gone through a second, or even third pass. I've heard it's reasonable to score between 40-60% on your first pass, especially if you haven't started any real studying.

I would recommend a different qbank instead of starting uworld right now. It seems like you need more content review which uworld is a little too advanced for that. Since it's still early for you, I recommend the kaplan step 1 qbank since it's easier and will help you start seeing how certain disorders present in certain ways. And once you've completed it, then return to uworld and you should of improved.

Meh step 1 was long ago. That need dissipates fairly quickly and turns into *shoulder shrugs at minutiae*
 
Sample size is pretty small. 150/2400=6%.

As someone else said I would consider going through another bank + doing Anki like a mad man. Your content is not there yet it seems.
 
Thanks for the advice. I will definitely try to start content review immediately as I agree there seems to be a major gap in my ability to recall information especially with regards to things taught in year one.
 
I have heard do kaplan first, hammer home the concepts, then save uworld for 6-8 weeks out when you focus on the rationale behind each answer choice (correct and incorrect)
 
I sucked at first too. Just keep plugging away and spend time reviewing the questions. Persistence.
