Title says it all, and I'm at a loss for what to do. I contacted my PD and let him know - wasn't of any help.

What can I do now? Do I lose at least one year?

You weren't able to find a prelim position (in anywhere for anything) through the SOAP? Unfortunate. As I understand it, if you don't have a PGY-1 year completed (basically if you don't start on July 1 or whatever) you're SOL. You will lose your advanced position entirely.

Yes, that's correct.

Is there any chance I can defer my advanced position and find a prelim next year?

Zero chance.

If you do not find a prelim in the next 2-3 months, your match will be waived and you will lose your advanced spot. Assuming you are not doing neurology, you should call up every single surgical program that does prelims in the country and offer them your firstborn child for consideration. Chances are, as someone with an advanced already lined up, you will be able to find somewhere that still needs a warm body (or can make a spot for one because they have extra funding/extra space in their cap.)

If you are doing neurology, do the same for medicine programs, but you will have a much harder time. It is a really tough position to be in.
