Matched TY without advanced position

Hello everyone, I am an Allopathic senior that matched into a TY program starting in July, but did not get an advanced position. I have average board scores, decent clinical grades.

What are my chances of getting into an IM position? Do I have to reapply through the match this year, or can I start looking for open positions? Will programs allow me in if I am already going to use up a year of funding?

Hey there. Many spots are able to offer PGY2 positions if the applicant is in a program that will fulfill year one requirements in IM and things go well. Your best bet is the institution in which you matched to a TY, so reaching out to your current program director and letting them know is a great first step. They may be able to alter your TY to fit more IM requirements in so you could actually start as a PGY2 the following year, though this is by no means guaranteed. If you want to do residency at another institution, you may have to apply through the match for a PGY-1 position, though some places do accept well qualified PGY-2 applicants outside of the match. Again, your program director should know early that you are considering IM as this will help them guide you.

Thank you for the reply. Unfortunately this TY has a lot of electives that may not fit into an IM scheme and it doesn't seem as though I will be able to change much of it. It seems that I will have to apply again in the next match. Not sure if I should just apply IM(PGY1) or also include and the original specialty (R positions).

Will IM programs be less likely to look at me given that I have already done a PGY1 transitional year?

Has anyone been in this position before and had it work out?

I'm not sure, but again, reach out to your program director! If it has a lot of electives, those electives may be changeable to fit into an IM scheme.
 
So it looks like IF I was accepted into a PGY1 year I would only get half of my funding for the final year of internal medicine. Will this make programs not even want to look at me?
 
Yeah, they asked what the requirements were (no idea how to find this out myself), but weren't optimistic about it.
 
Well, then you'll have to work with what you have. The good news is that you have a first year residency position, so you'll have a great explanation for the year you spent, and you'll come out with some extra training. I'm sorry that it's not working out exactly the way that you planned, but an extra year in the grand scheme of things will not ruin your life. I did an extra year doing med-peds instead of medicine and now regret it, but I still get to be an attending when I'm done.
 
Anyone know what will happen with funding? Will programs be less likely to take me because of this?

Anyone with experience of reapplying to internal medicine after a pgy1 year?
 
Close friend matched into TY and advanced program. At the end of her TY year realized she wanted to do IM and not her advanced program. So she reapplied to IM programs for a PGY1 spot and matched into her top choice. This was ~10 years ago.

A lot of programs will take PGY1 applicants who have done 1-3+ years of a different residency, so by no means is it unheard of. The program just needs to not run such a lean margin that they can't afford to fund a year or two of someones training.
