This is a continuation of my prematch dilemma. Do you guys think I will drop down the list for a program if I reject a prematch offer from them? I know that people have given their opinions on this in previous threads but does anybody know of actual instances of people actually matching or not matching to the same program?

My opinion is that yes your rank will drop. By rejecting the offer you are telling the program that they are not your number 1 or probably even top 5 or you would have accepted the offer. why would they rank you highly knowing you dont want to go there. They basically call your bluff.
I believe that PDs really want to know who is coming to the program so they want to turn in a ROL that is composed of applicants who want to go to their program.

I think you need to do some work. First figure out if this particular program only/mostly fills its positions via pre-match. If answer is yes, then perhaps you are out of luck because they might offer same pre-match slot to someone else. Remember sometimes programs will tell you, that you can have 3 days to decide but at the same time they don't wait for 3 days. Do you know how many FMG/IMG are chasing same slots. In general programs can always find competent person to fill their positions. I would assume when you applied for residency or when you got your interview calls, you should have known where do you stand and what to do if you get a pre match offer. Were you clueless 4 months ago? So moral of the story, if you are uncertain then seriously consider accepting pre-match offer.

Now 2nd situation. Programs do not have all or none criteria. That is they have to take best person among all applications. So it is not they have to wait for an ideal candidate. If you are NOT going to accept their pre-match offer then I assume they would know that you have other programs at the top your list. No program going to tell you, gee go ahead and put us at the bottom of your ROL and we will be there for you. Moral of the story is, it does not cost programs any additional money to rank all of people whom they have interviewed or few. So you will be ranked but I would guess in this situation you may not be at the top of their list.

If program occasionally offers pre-match then I think might have a chance as you don't know how other people going to rank this program. Lets say if you are talking about university based program in South Dakota then I would think perhaps you are not the only one being turned off by their location.

This is a program in NY which only offers 5-6/33 spots for prematch. I like the program but never expected a pre match from them. That's why the sudden dilemma...

You have already mentioned it is university program and now you telling strength of program being 33 in PGY1. It looks like a big program. You need to look what do you want in your life. Fellowships or good job after residency. In general being in University program will help but it will also depend upon other factors like your evaluations etc.

NY city and surrounding areas are very desireable even upstate NY is not bad. Lots of people will take it if this is in or near NY city. Really it is your choice and I don't think any one can help you like what to do?
