Transcript matching form for universities you attended?

Yes, saved it as a .pdf.

Go to the main menu and go to Colleges attended. Then in there there is a column that says "transcript matching form" it is next to the institution.
Under it there is a little orange circle if you click it, it gives you the .pdf.

You have to click on "add new entry" or whatever at the top and add your institution. Once you have added it then it a little orange circle will show up next to your school name and when you click on it then you can download the matching form.

is anyone else form for the question of whether it is for a grade change or not blank?

As in there's is no box or line to put an answer?
