McMicro Question

Alright...
Wasn't too thrilled about Micro/Immuno at our school and starting reading CMMRS...(ridiculously simple)

Glancing over the charts right now and wondering,

(A) as far as mechanisms go, i.e. vibrio cholera, is it enough to know that increasing cyclic AMP levels result in increased intraluminal NaCl---> diarrhea?

or

(B) Do I need to know (detail-level-wise) that there are five B subunits and then two A subunits carrying out the ADP ribosylation of the GTP-binding protein which activates adenyl cyclase and ultimately leads to the cAMP increase?

I'm guessing choice "A?" I don't have any test banks or NBME q's yet, so not sure how in depth I need to go with micro bug mechanisms...

Also, since I DON'T have any qbanks, anyone care to comment on whether USMLE World is enough to do better than avg or Kaplan qbank is necessary as well? Seems like Kaplan maybe updated their qbanks to follow the computer format of the exam better in recent years? I still heard the questions are nit-picky and frustrating...Votes?

Actually it's a mix of both. You'll need to know about the ADP ribosylation of Gs, and the mechanism of how G(s) protein coupled receptor pathway works leading to an increase in cAMP via stim. of adenylate cyclase. As far as the subunits are concerned, I'd save that for last minute review of minutiae, etc, after covering the basics.
