MD/MPH programs

I am thinking about applying to some of the dual degree programs for MD/MPH vs regular MD. Specifically I am looking at UConn, Loyola, Saint Louis Univ, Case Western, and OHSU. I also think the PRIME-LA program at UCLA Geffen is pretty cool. Since these programs only allow a few students in, do they usually consider applicants for the regular md if they dont get into a dual degree program? I am worried that if this decreases my chances of being accepted at all I shouldn't apply for them. Thoughts/experiences with this?

Most dual programs will either automatically or offer you to be rolled over into the regular MD if you are initially rejected from the dual
Do most schools let you apply for the dual-degree program after acceptance? Or even consider people after matriculation?
