If you get accepted to an MD/PhD program, can you choose to only do an MD after?

There are often stipulations. Sometimes you gotta pay everything back, sometimes you don't. Programs are specifically looking to weed out folks who will do this though, as it can be costly to the program. It should be a peace of mind type thing, not a possible out.

Acceptance to an MD-PhD program implies acceptance to the MD program.
If you're asking can you switch to an MD in the middle of your education, you can do that as well, but you may have to give pay back your tuition support (I am not sure on that part).

Yes, you can choose to abandon the PhD at any time without being removed from your medical school. Whether or not you have to pay back your tuition and stipend money depends on the school. Programs funded by the government as MSTP programs cannot force students to pay back that money. Other, privately funded programs can ask you to pay back as much of it as they want.
At many schools, yes. It is not uncommon for MD/PhD students to receive a tuition waver, a monthly stipend for living expenses, and free insurance coverage. However, programs vary on how much they will cover. Some do not provide free health insurance, and some do not provide a stipend or tuition waver during the medical school years.
