Basically as the title says, I am an MD re-applicant but first time DO applicant, I know it sounds silly but I want to make sure this mean I can tell my DO admission committees I am a first time applicant? I'm worried it seems I'm only applying DO as a backup but after a lot of reflection, I'd be truly happy practicing both !

One or two of the DO secondary apps I filled out last year asked about this specifically. You need to come up with a reason that isn't "I didn't get accepted to MD." I can't remember which schools.

In my experience working on secondary applications most schools asking if you are a “first time applicant” are asking if you have applied specifically to their program. Since you haven’t applied to any DO schools before you would be a first time applicant to those specific programs. That being said there may be schools that ask more detailed questions about your application history.
