Some places use techs, nurses, or pharmacists.

ASHP has a goal for pharmacists to do them. Do you think this would be a good thing?

Right now it's done by nurses at my hospital, and the mistakes, misspelling and other dumb **** I see every day is terrible. Our DOP told us that once we roll out the CPOE and EMR system, we're gonna be on the floors doing things like med recs, discharge counseling, etc.

It's obvious pharmacists are better equiped to do med rec.
