At my school the "Sheets" just contain the required questions each applicant gets asked as well as the obvious categories that should be addressed during the interview.
I am not aware of any school using a "checklist" or any form that could really be of any use to a student interviewing.
Also if any of these sheets were to get posted here it I'm sure the schools involved wouldn't like it too much.
Basically there are no magic words you have to say to get into medical school. Just be able to express yourself well, and have some sweet interesting experiences to discuss that show why your better than the other 30 people interviewing today.
