Medical documentation question

Hi guys, what type of medical records I have to submit if admitted to dental school and when? Thank you!
 
Vaccine records(MMR, Varcella, etc) and a physical. My physical is due before June 1st and now I'm waiting for my 2nd MMR vaccine because I was surprisingly negative for antibodies.

What is physical, is it basically a letter from the doctor? I don't understand what should be in it? What is it?
 
Yes, you set up an appt at a dr for a physical. They just make sure everything is working correctly. Be prepared to have you balls touched, so don't go in with an afro.
 
Usually you have a specific form from the school where you have been accepted, it comes with the acceptance package. This form will detail the kind of vaccinations you must have or must get before school starts, e.g., MMR, Hep A, Hep B, Tuberculosis, meningitis shots, etc. You take the form to your regular primary care physician office (where your vaccination documentation is kept) and have your doctor fill it out and sign. He/she will order the shots you haven't had. I had to get a TB chest x-ray too, because 2 of my schools (one where I got accepted, the other one where I was waitlisted) require it. Some schools require your checkup exam to be no more than 6 months before matriculation, so don't get your annual exam too early (March or later in the year that you start dental school to be safe).
