This would be a clear violation of the Americans with Disabilities Act of 1990, so I highly doubt that it has a negative impact on applications.

Some of the adcom members here can probably provide additional reassurance.
 
I had two semesters of full semester medical withdrawals. My university, after obtaining documentation, has clearly marked these semesters as medical withdrawals. They are two completely unrelated conditions—I just got unlucky. I am concerned that medical schools will view this as me being too ill for the rigor. Also, should I provide documentation to medical schools?
You don't need documentation. Say that you had a health issue that is being tx'd and has been resolved

Your academic excellence will allay any worries that Adcoms might have.
