Most counties are underserved which does not mean that they are poor but that they don't have enough primary care docs. Even Manhattan with four medical schools and plenty of millionaires is (U) on the AMCAS application which is accurate if you've ever looked for a primary care provider in many NYC (Manhattan) neighborhoods.
