medicine LOR for prelim year

So now that I've sent out my ophtho application, I'm ready to tackle the ERAS app for prelim/transitional yr, and of course I haven't even looked at it yet, nor have I thought about LORs. I just heard from someone that for a medicine prelim, you need a medicine "departmental letter" for a lot of the programs. Is this true? I already have a medicine letter I can use, but a departmental letter from the director of the medicine rotation (which is written kind of like a dean's letter and quotes residents and attendings) is kind of a pain to get, especially at this point. Obviously I realize I can contact the programs I am interested in and ask about this, but I am hoping someone here may already know about this and can save me a little effort.
 
I matched at a pretty competitive prelim IM program without a "departmental" letter.
